Location:        In-person at West Toronto Youth Hub (Jane Street Hub, 1541 Jane, 2010  Eglinton Avenue West)

Yorktown Family Services provides a comprehensive range of mental health services for children, youth and families who are experiencing serious emotional/social/behavioural difficulties along with several gender-based violence initiatives. Yorktown is currently seeking a Wraparound Facilitator who will join the team of professionals at Yorktown.

Primary Function
  • To provide facilitation of the Wraparound Process with individuals, children and their families
  • Preparing individuals/families for child and family teams, encouraging the principals of access, voice and ownership 
  • Developing Wraparound and Crisis Plans, monitoring the follow through of the plans, updating/altering plans on an as needed basis
  • Promoting Wraparound within the agency and the community
  • Peer support membership and participation (including WAO membership)
Responsibilities
  • Facilitate the Hi-Fidelity Wraparound Process within the context on an Individual and Family Team (IFT)
  • Manage and respond to referrals from Black Lives Assessing Care (BLAC)
  • Develop and identify a Wraparound team of supports in collaboration with the youth and family and orient them to the Wraparound Process and philosophy
  • Mobilize formal and informal supports and develop a transition plan for youth and families that includes formal and informal supports
  • Maintain and nurture partnerships with CAST, CCAS and other partners
  • Resolve issues with others in a manner that is respectful, cooperative and in accordance with the agency culture and philosophy
  • Encourage the principals of Access, Voice and Ownership for youth and families
  • Complete a Strengths, Needs and Cultural Discovery Assessment with each Wraparound family and their team
  • Develop, implement, monitor and revise Wraparound plans, including Crisis Plans
  • Ensure the mandate of the Child and Family Services Act is addressed in the family plan as necessary
  • Record notes, statistics and other details as per agency policy in a timely manner on electronic database
  • Drive the process forward in a timely manner while modeling the facilitation role within the team
  • Participate in ongoing training, monthly peer support meetings, WAO meetings and other meetings and networking opportunities as necessary
  • Participate in regular supervision with Manager of West Toronto Youth Outreach Network and Community Services
  • Work flexible hours in order to meet the demands of direct service requirements
  • Balance the demands/priorities of workload, manage time effectively and operate within the framework of stated expectations
  • Exercise professional judgement in conflictual and stressful situations
  • Strict adherence to agency policy and procedures
  • Report any violations of the Act s/he becomes aware of to a supervisor
  • Strong awareness of duty to report procedures
  • Participate in agency committees, supervision of placement students, staff development and researching topics of mutual interest and concern
Qualifications
  • Child and Youth Worker / Social Service Worker Diploma, Degree or equivalent with 3-5 years’ experience working in Direct Service with children, youth and Families. 
  • A strong and demonstrated belief in children and families, experience that supports a community minded to service
  • A comprehensive knowledge of the social service sector, as well as systems; how they work, the barriers facing youth and families and system navigation within the greater Toronto area.
  • Sound knowledge of formal and informal resources in the community
  • Leadership experience within a community-based organization
  • Ability to collaborate effectively in a team as well as work independently.
  • Hands on experience within a community-based organization
  • Proven ability to apply a strength/solution based and client centered approach to work with children and families
  • Strong organizational and time management skills
  • Will have completed Hi-Fedelity Wraparound Facilitator Training or willing to participate in this training
  • Ability to coordinate services and case manage with other programs while maintaining effective working relationships
  • Culturally competent and demonstrated ability to work with culturally diverse populations in an effective manner (anti-oppressive, anti-discrimination, anti-racist approaches)
  • Vulnerable Sector Screening is required
