Community Living Mississauga has been a source of support for people who have an intellectual disability since 1955. Community Living Mississauga provides a variety of supports and services to people who have an intellectual disability, including: residential supports, employment resources, community engagement, Supported Independent Living, respite, and various day support options. Description of Position

Working under the guidance of a Supervisor or Manager, this position is responsible for individualized service coordination support for people who have an intellectual disability and their families.


Description of Duties

  • Provide service coordination support based on goals identified by people supported and their families.
  • Make appropriate referrals to services and programs in the community to address the active goals identified.
  • Keep informed about and utilize the resources available in the community.
  • Promote community inclusion, independence and self-determination for the people supported.
  • Work collaboratively with co-workers, community partners, the people supported and their families to solve problems and resolve conflicts.
  • Promote the standards set by the organization in terms of Personal Outcome Measures and incorporate these standards whenever possible when planning with families.
  • Identify/Report support situations that do not meet the “Minimum Standards Guidelines” set by the organization.
  • Utilize/Develop new techniques, options or approaches to supporting people and their families.

Qualifications

  • Developmental Services Worker Diploma (preferred) or Social Services Worker Diploma, Child and Youth Worker Diploma with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
  • Will also consider a University Degree with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
  • Must have a valid Ontario Driver’s License (Class G or G2), access to a reliable vehicle and Insurance.
  • Current valid First Aid/CPR certificate required.
  • Proficient with PC computer environment; MS Office (Word, Excel, Outlook, etc.) and other software.
  • Excellent interpersonal and communication skills (verbal, non-verbal, written).
  • Ability to work collaboratively in a team environment and with families as necessary.
  • Good problem-solving and conflict resolution skills.
  • Bilingualism in English/French or other language would be considered an asset.
