To help fulfill its mission, LOFT is hiring a Psychogeriatric Case Manager.


Job Summary

As a Psychogeriatric Case Manager you will be part of the @Home Team provides
direct care, education and support to older adults and their care partners, who are
demonstrating responsive behaviors that may be associated with various types of dementia,
mental health, substance use or other neurological conditions living within the community
within a specified geographical boundary. You will complete a comprehensive behavioral
assessment and an evidence-based Behavioral Support Plan. You will assist in the creation
and coordination of a support network through establishing linkages with other community
agencies and practitioners to coordinate care for the clients. In addition, you will provide
direct service to support the client, family members and/or care-givers to mitigate
escalating responsive behaviors. You will also liaise, collaborate and communicate with all
members of the Behavioral Support Services team and other partners within the circle of
care.


MATCH is a specialized team of providers from LOFT and Inner City Health Associates (ICHA), that delivers services to the most vulnerable individuals to assist in accessing and maintaining housing and connecting to health and other supports in the community. The MATCH team consists of 11 case managers, 2 registered nurses (shared), 1 peer support workers and 2 part time psychiatrists (ICHA). MATCH provides clinical hands-on support to those with complex healthcare needs through case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy. MATCH works with individuals who have previously and/ or are currently experiencing homelessness, whose health and needs are stabilized while working with the CATCH and M-DOT teams.  MATCH provides long term multi-disciplinary wrap around and case management and health services in their (often new) communities. The aim of MATCH is to improve transitions of care for individuals and works with them to identify and meet their goals toward recovery, while gaining stability and building community


What You Will Do:
  • Assists persons served in the development and implementation of a Behavioural Support Plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours.  Identify and refer to resources specific to those needs and issues;  
  • Act as a liaison and advocate for the person served among those involved with the individual’s care including family, other care-givers, medical and other multi-disciplinary providers;  
  • Complete PIECES/Functional Assessment and provide timely and accurate record keeping and reports as required;  
  • Provide  support, on-going training and  consultation to colleagues, family members and/or other care-givers to assist in mitigating escalating responsive behaviours;  
  • Provide supportive counselling or referral and linkages to health care, programs or service providers; 
  • Provide crisis management/intervention and assist in the development of crisis plans for persons served;  
  • Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation principles;  
  • Maintain strict confidentiality and follow all other required policies and procedures;  
  • Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;  
  • Effectively participate as a member of the Behaviour Support Resource Team; attend staff/team meetings,  care conferences for the person served, other multi-disciplinary meetings, committee meetings etc as required;  
  • Attend and actively participate in all required training;  
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services;  
  • Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption pursuant to the Ontario Human Rights Code in accordance with LOFT’s Communicable Diseases policy and program guidelines  
  • It is an expectation of the position that most of the work will be conducted in hospitals and compliance with their occupational health polices and protocols will be required (associated costs will be reimbursed)  
  • Complete other duties, from time to time, as assigned by the Coordinator and/or Program Director.

What You Bring:
Must Haves
  • Completion of  a diploma/degree in Social Services/Human Services or equivalent;
  • Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with persons served on goals/priorities;
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills;
  • Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive training;
  • Experience/familiarity with RAI documentation, and OCAN and health profession standards for documentation of information of the person served;
  • Ability to collaborate effectively with other members of inter-professional care teams, persons served, family members and community team members;
  • Good oral and written communication skills and computer literacy, including MS Word and Excel;
  • Counselling and responding skills;
  • Creativity and flexibility to approaches;
  • Knowledge of community resources and supports;
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies;
  • Strong assessment skills;
  • Understanding of cultural competency and framework
  • Valid drivers’ license and access to own vehicle is required;
Nice-to-Haves
  • Physically able to perform demanding body mechanics (i.e., lifting, transferring, etc.)
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
  • Ability to communicate in a different language, an asset.
