To help fulfill its mission, LOFT is hiring a Nurse Practitioner.


Job Summary

The Nurse Practitioner (NP) is a primary care provider to clients and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario. The NP is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the integrated care team. The NP collaborates with the client and family/caregiver, and the health care team in the development, implementation and evaluation of the client’s plan of care; provides leadership and mentorship to program staff that enhances their knowledge, assessment skills, and ability to care for seniors in place. The NP will be responsible for working with individuals from diverse backgrounds, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of adults and seniors in a way that enhances their dignity and personal empowerment. Additionally, this role will liaise with community agencies, other practitioners, and key partners, including care partners, to coordinate care for clients.

Social Medicine Initiative is an interdisciplinary team of providers from the University Health Network and LOFT Community Services that provides wrap around services to individuals with concurrent conditions that span physical and mental health challenges and social and economic vulnerability who are experiencing homelessness or at risk of losing their housing.  Social Medicine delivers clinical hands-on support to those with complex healthcare needs providing outreach and case management services with access to psychiatric and medical assessment, consultation, and access to treatment, service planning, advocacy, and system navigation support. The aim of this partnership is to provide low barrier access to housing for these individuals, while assisting in stabilizing their physical and mental health, substance use, behavioural, psychosocial, and social drivers of health challenges. The Social Medicine team works from a housing first and harm reduction approach. The goal is to work with individuals in getting connected to housing and follow up support that meets the level of need required for each individual to live successfully in the community.


What You Will Do
  • Providing comprehensive primary care to clients as part of the interdisciplinary health care team
  • Modelling excellence in nursing practice through the utilization of advance concepts related to psychogeriatric care, specifically in the areas of assessment, monitoring, planning, implementation and evaluation
  • Providing leadership and direction to the team in resource planning, health system improvement along with professional and program development, implementation and evaluation
  • Providing expert support and consultation to facilitate system change
  • Participating in decision making regarding resource utilization, in collaboration with the management team
  • Developing and revising clinical guidelines aligned with current evidenced-based best practices
  • Provides consultation around complex symptom and behaviour management in seniors living with mental health challenges
  • Acts as a change agent through knowledge translation and dissemination
  • Provides health teaching to clients and family members concerning medication, illness, symptom management, and recovery using a biopsychosocial framework;
  • Provide advice, coaching and support to staff regarding medical/healthcare questions and concern
  • Provide support with behaviours arising from cognitive issues, and contacting appropriate emergency services in crisis situations;
  • Monitor client’s symptoms and initiate interventions to facilitate stabilization when required;
  • Maintain all records of service and ensuring supplies and equipment are readily available;
  • Conduct mandatory assessments and provide timely and accurate record keeping and reports as required
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures;
  • Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required;
  • Liaise, collaborate and coordinate with other professionals, service providers, CAMH and UHN care partners, and other care partners as needed
  • Follows standard infection control practices and enhanced precautions as instructed

What You Bring
Must Haves
  • Registered with the College of Nurses of Ontario as an Extended Class RN and is a member in good standing;
  • Minimum 5 years of experience working with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges;
  • Membership in the Nurse Practitioner Association of Ontario preferred;
  • National certification in Gerontology Nursing from the Canadian Nurses Association preferred;
  • Advanced clinical knowledge of current best practices in psychogeriatric assessment and treatment planning required;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Experience in Case Management and Care Coordination preferred;
  • Knowledge of social services and community resources and supports for older adults;
  • Current BCLS/CPR certification required;
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
  • Knowledge and experience working with relevant legislation (i.e. SDM Act, POA, Consent and Capacity, etc.)
  • Advanced nursing assessment skills and sound knowledge of clinical interventions for geriatric population;
  • High level of initiative and self-direction;
  • Exemplary interpersonal and communications skills;
  • Self-directed, results oriented and capable of working independently as well as in a team environment;
  • Excellent oral and written communication skills and computer literacy;
  • Well-developed problem-solving abilities and interpersonal skills;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Demonstrated ability to provide individual/group health teaching for residents and staff;
  • Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
  • Demonstrates awareness and understanding in working with individuals from different cultures;
  • Demonstrates initiative and creativity in the workplace;
  • A positive attendance record;
  • Criminal Records Check and Vulnerable Sector Screening;
