Manager, Patients and Caregiver Support - Crohn's and Colitis Canada

IDEAL CANDIDATE

This is a meaningful role where you can make a difference every day for people living with inflammatory bowel diseases (IBD). Our ideal candidate is a passionate social worker and a strong communicator with excellent program management and patient support skills to lead Crohn’s and Colitis Canada's patient and caregiver support programs. The manager will be utilizing an interprofessional, biopsychosocial and patient-focused collaborative approach in supporting trial participants transition from pediatric to adult care hence an understanding of biopsychosocial model of care and/or health services research is an asset. The candidate will have the ability to exercise good judgment in a variety of situations and be able to balance multiple priorities. Moreover, the ideal candidate should possess high emotional intelligence and be passionate about improving the lives of those affected by IBD.

QUALIFICATIONS

  • Master’s degree in social work or related field;
  • Minimum of 3 years of professional experience in the areas of emotional and mental support, ideally of people with chronic diseases;
  • 1 year of experience developing, managing and evaluating programs, data design, collection and analysis, and people management;
  • Experience in case management, assessment and support transition age youth in multi-disciplinary setting;
  • Experience and in-depth knowledge of government policies and regulations related to support programs;
  • Understanding of the potential risks with offering support programs and best practices to evaluate and mitigate risks;
  • Possess sound judgement to manage challenges in successfully delivering on program milestones;
  • Ability to synthesize and apply research evidence to programs and ability to develop business cases to identify the best course of action in assessing and developing programs;
  • Superior interpersonal and communication skills (oral and written), including the ability to act with tact, diplomacy, and professionalism, and handle sensitive information with discretion;
  • Proven ability to work constructively and professionally with diverse groups of stakeholders, including physicians, scientists, donors, industry representatives, staff, and patients;
  • Excellent organizational and time management skills, including the ability to organize workload, set priorities, and respond effectively to competing demands; and
  • Successful completion of a criminal background check.

RESPONSIBILITIES

  • Lead the design, development, implementation and evaluation of all Crohn’s and Colitis Canada’s virtual and potentially in-person support programs;
  • Incorporate results of data into the design and development of implementation plans for each support program for the upcoming year(s), including assessing the ROI of updating the program;
  • Develop and track program budgets;
  • Ensure effective collaborations cross-departmentally and, if needed, with external stakeholders to support the success of the programs, including promoting and raising awareness;
  • Design, develop and implement evaluation into all programs as well as SOPs to ensure data quality, integrity, accuracy and fidelity;
  • Actively manage youth participating in a pilot research project on transitioning youth from a pediatric to adult care models, which include biopsychosocial assessments, case documentation, and study related meetings with the multidisciplinary research team (~ 10% of work hours);
  • Works with members of the Development team to develop funding proposals;
  • Supports the VP Research & Patient Programs in developing funding proposals and final reports to industry;
  • Identify opportunities to educate Crohn’s and Colitis Canada’s employees on the support programs;
  • Coaches and mentors direct reports by identifying skill strengths and areas of improvement for individuals.
