Salary: F - $50,182.70

What You Will Do
  • Conduct tenant needs assessments and create care plans for individuals who have current or past hoarding tendencies and /or limitations for unit maintenance. Engage the tenant in identifying any concerns or challenge areas inside the unit for creating a healthy living environment.   
  • Encourage and support tenant engagement in the ongoing management of their unit to achieve positive outcomes related to decluttering, garbage removal, laundry, food safety, functioning fire detectors and preparation for pest related needs/treatments. Collaborate with the Support & Housing Team and/or exterior contracts for the disposal of items from the unit, where possible.
  • Coach and educate tenants on their responsibilities as per the signed tenancy agreement with WoodGreen Housing. Work with tenants to ensure a clear understanding of housing concerns as related to fire safety with hoarding and /or participation in pest control treatments.
  • Identify resources and develop collaborative plans with tenants with respect to issues of hoarding behaviour. 
  • Work with and educate tenants to understand the relevance of healthy living standards
  •  Ensure tenant ownership and empowerment through processes of upkeep, decluttering and overall for both s/he and fellow tenants.
  • Adapt and evaluate decluttering plans and programs on a regular basis while working with tenants to meet changing needs
  • Collaborate with support teams, management team and housing staff to plan and facilitate needed outcomes
  • Help tenants to set and achieve realistic goals with respect to bringing their units to an acceptable safety standard.   Help tenants to achieve set goals through supportive means such as encouragement, coaching, psycho-socio education, and de-escalation.
  • Attend Hoarding Network and other community-related meetings to network and keep up to date on best practices for hoarding supports and follow up for behavioral issues that impede progress.  
  • Liaise with management team and support staff team to coordinate and provide information sessions for participants within portfolio.
  • Provide & seek support from Clinical Counselor/Case Managers with respect to crisis intervention and conflict resolution as required.
  • Monitor and review tenant needs, building concerns and community issues and communicate the information to the management/support/housing team
General Responsibilities
  • Carry out administrative procedures and maintain appropriate records with respect to day to day interactions & interventions.
  • Uphold confidentiality policies regarding tenant information.
  • Prepare statistical and narrative reports and updates of the interventions for the management team
  • Responsible for upgrading training and education as directed by WoodGreen Community Services to fulfill the responsibilities of the job.
  • Ensure the anti-racism objectives and principles of the organization are achieved within the program.
  • Responsible for representing the unit on internal staff committees as directed by the management team
  • Participate in regular supervisory sessions with the management team.
  • Participate in regular staff and unit meetings. 
  • Be prepared to work in environments where there may be death, dying, drug use, smoking, odours and inappropriate language.
  • Perform other related duties as required
What You Bring to the Team
  • Post-Secondary Degree in Social Work or related field; or Diploma in Social Services with several years of related experience
  • Minimum 2-3 years direct experience in social services working with individuals who have history of homelessness, low income, addictions and mental health challenges
  • Comprehensive knowledge of bed bug and other pest concerns, pest control procedures and solutions
  • Comprehensive knowledge of best practices for hoarding support and follow up
  • Demonstrated interpersonal and team work skills; ability to work cooperatively with team members
  • Self-motivated with the ability to problem solve individually, and as part of a group
  • Positive and goal-oriented; understands that the change process is both labour and emotionally intensive.
  • Understand resistive behaviour and apply relevant strategies to move forward
  • Demonstrated knowledge, understanding and experience with working with individuals who may have experienced violence, addictions, abuse and /or homelessness.
  •  Demonstrated cultural competency with the ability to communicate and engage with individuals from diverse ethnocultural backgrounds and levels of abilities.
  • Demonstrated knowledge & experience with needs assessments and coordinating services and implementation. 
  • Excellent oral, and written communication and documentation skills
  • Strong ability to work collaboratively in a team environment or individually, to multi-task, organize and prioritize efficiently
  • Demonstrated knowledge of client-centred, anti-racist, and anti-oppressive frameworks
  • Demonstrated experience in crisis response and intervention, de-escalation, advocacy and negotiation skills.
  • Comfortable working in an environment in which clients may be angry and exhibit anti-social behaviours.
  • Applied computer skills (MS word, Microsoft Office 365 /Teams, Excel).
  • Experience with Pirouette database is an asset.
  • A second language is an asset.
  • Must be able to lift up to 40 pounds.
  • Able to work in settings which are crowded and may include secondhand smoke and/or unpleasant odours.
