Applications invited for: Foundations Mentor, Temporary Full-Time – One Year Contract
The position involves providing direct support to adults with developmental disabilities supported in the Foundations program in Perth County. The Foundations Mentor will support individuals to create a life of meaning, belonging and choice after high school, with a unique focus on assessing support needs, self-awareness, leadership, self-advocacy, community involvement, health and safety, life skills, building informal support networks and future planning.
This position is for 35 hours/week and may involve some evening and weekend support as needed.
Qualifications: Developmental Services Worker (DSW) diploma preferred or an acceptable equivalent with strong background in Developmental Services. Consideration will be given to those with a complimentary degree or related diploma from a recognized institution.
Requirements include: Reliable vehicle with Valid “G” driver license with $2 Million liability coverage as travel is required; Willingness to work occasional evening and weekend hours, as needed.
