Program Overview – First Step to Home, 650 Queen St. E, Toronto, Ontario

First Step to Home (FSTH), 650 Queen St. E, Toronto, Ontario First Step to Home is a transitional program with housing as an enabler for men 55 years and up who have been street involved and experiencing mental health and/or addictions issues. The program provides onsite psychosocial primary care supports and services including intensive case management; harm reduction and addiction supports; therapeutic and supportive counselling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

FSTH is the primary location, however the FAC will assist with the food program at the street to home programs as well, i.e., 841 Queen Street East and 1117 Gerrard Street East. The Street to Home Supportive Housing Program offers housing for individuals facing significant vulnerabilities due to homelessness, mental health challenges, and/or substance use disorders. Similar to FSTH, the program delivers onsite psychosocial and primary care services, including intensive case management, support for harm reduction and addiction, therapeutic counseling, assistance with daily living activities, life skills development, opportunities for social and recreational engagement, volunteer programs, and ongoing follow-up support.

What You Will Do

• Conduct assessments to determine individual nutritional needs and develop an individualized plan
• Implement food services program within budgetary constraints 
• Coordinate, monitor, and ensure the proper ordering, receiving handling, and storage of both purchased and donated food items.
• Assist with the food programs at FSTH and Street to Home.
• Maintaining and cultivating strong relationships with food banks and other supportive agencies. This includes networking and conducting outreach to a diverse range of food providers.
• Coordinate and operate food banks and similar programs to ensure access to nutritious food for tenants
• Develop and maintain food related projects
• Support front-line staff in the supervision of volunteers and placement students
• Provide direction and coordination of team development needs and opportunities/initiatives
• Maintain a safe and welcoming environment through excellent customer service and conflict management
• Provide one-on-one coaching to tenants on nutrition, meal planning, and grocery shopping
• Develop, enhance and maintain systems including all records and schedules as required
• Tracks daily meals served and other relevant statistics, and submit report to management, for funders and program development with the assistance of Food Access Assistant and Drop-In staff
• In conjunction with the Director and other Drop-in Coordinators, monitor and ensure the continued development of program policy and procedures
• Develops and maintain strong working relationships with other internal and external services and programs
• Ensure the purchase and maintenance of program equipment and supplies
• This position will include some evening and weekend shifts monthly and is subject to exposure to second-hand smoke, odors, pests and clients’ potential use of inappropriate language or swearing.

What You Bring to the Team

• Bachelor’s in Nutrition, Public Health, a related field or work experience
• 2+ years’ experience working with individuals mental health, addiction, street-involvement, and community based nutrition programs, preferably seniors
