Overview

This program aims to increase access to HIV, Hep C and harm reduction information and resources for federally incarcerated people living with and at risk of acquiring HIV and Hep C In addition, program staff provide l case management support and advocacy to increase access testing, treatment and support. The coordinator will work with community partners to create accessible, culturally safe pathways to healthcare and social services.

Job Duties:

• Coordinates and implements prison in-reach programming on topics including overdose prevention, safer drug use, safer sex and HIV, Hep C and STBBI
• Provide case management support and advocacy for prisoners/ex-prisoners living with and at risk for acquiring HIV and Hep C
• Provide reintegration, system navigation support for prisoners/ex-prisoners living with and at risk of acquiring HIV/Hep C prior to and upon their release for prison.
• Foster service partnerships and networks with diverse community stakeholders to increase access to HIV, HEP C testing treatment and resources for currently and formerly incarcerated people
• Develop relevant educational resource materials in collaboration with federally incarcerated people
• Support the development of stigma free, culturally safe services for prisoners/ex-prisoners through the through delivery of capacity building service provider training.
• In collaboration program manager develop and maintain relationship with relevant correctional staff
• In collaboration with the Program Evaluator develop tools to evaluate to measure program impact
• Participate in regular supervision and team meetings, and agency events.
• Maintain records, reports, and statistics necessary for program functioning, monitoring, evaluation, and reporting
• Attend program, department and agency meetings and trainings.
• Perform other duties as required

Qualifications:

• Demonstrated knowledge of the structural, economic, social, and political contexts that affect both incarcerated people and those who were previously incarcerated in Ontario.
• Knowledge and application of cultural safety principles, trauma-informed approaches, and an ability and willingness to work closely with communities who are over-represented in the carceral system
• A strong commitment to advancing justice, equity, accessibility and inclusion both internally and in the way programs, and other activities are designed and led Understanding of the health and social impact of the criminal justice system on individuals and society and experience working with HIV and Hep C issues including a well-developed knowledge of relevant resources
• Demonstrated experience with case management
• Experience in working with people vulnerable to incarceration, prisoner/ex-prisoner communities.
• Understanding of Hep C, drug use and harm reduction in prison context, especially in communities who are disproportionately criminalized.
• Strong knowledge of harm reduction and opioid overdose prevention strategies.
• Ability to determine needs, develop programs and facilitate community initiatives in collaboration with prisoners and ex- prisoners.
• Experience developing, facilitating, and evaluating targeted education and training workshop.
• Demonstrated knowledge and experience of harm reduction principles and practice.
• Expertise in community development focusing on marginalized communities.
• Experience working within an anti-racist / anti-oppressive framework
• Demonstrated skills preparing presentations, reports, program plans, educational materials, brochures, etc.
• Ability to develop and sustain partnerships, coordinate networks and support advocacy strategies
• Demonstrated ability to work under pressure meet deadlines and ability to work both independently and within a team structure.
• Demonstrated ability to anticipate problems/conflicts and take appropriate actions, to organize and prioritize work.
• Sound decision-making and judgment skills with the ability to effectively manage communications with community, committees, and coalitions.
• Demonstrates a commitment to a healthy and safe workplace for self and others by taking all reasonable precautions and working in compliance with PASAN’s related policies, health and safety legislation and best practice.

Position Requirements:

• Ability to obtain clearance into Federal correctional institutions
• Ability to travel and be on the road for multiple days at a time
• Driver’s License
