Location:         In-Person and Virtual at Primary Health Care locations

This is a mobile integrated care, behavioral health, social service support, and community capacity building model that provides direct service intervention and prevention for youth and young adults involved with or at risk of targeted violence acts across the Greater Toronto Area. The model involves numerous evidence-based and informed practices to divert youth and young adults away from targeted violence and or violent ideologies. This is a unionized position.

Primary Function
Requires a highly skilled Family & Individual Therapist to apply evidence based and evidence informed therapy services for youth, young adults, adults, and families who are impacted by or involved with ideologies that are disruptive to daily life or may lead to acts of violence.

Responsibilities
  • Oversee the treatment plan and coordination of services related to mental health interventions
  • Be the clinical expert in the area of mental health as part of a wider inter-disciplinary team
  • Work with families or family members through family therapy to engage members that may align with ideologies that are disruptive to day to day living or lead to potential violence
  • Work with closely with the Engagement and Intervention Support Worker to coordinate treatment plans and implement congruent interventions
  • Monitor on a regular basis through the use of various tools and screeners social engagement and mental health functioning
  • Complete psycho-social assessment reports, mid-term reports and closing reports
  • Provide therapy in person, over the phone, in the community or online
  • Meet with program participants up to 2-3 times per week. Low caseload and high engagement
  • Make referrals to more intensive services if required such as psychiatry, outpatient or inpatient support, residential care
  • Chair case consultation meetings weekly and monitor progress of all clients
  • Be able to rapidly respond (24-48hrs.) to referral requests or coaching skills in the moment
  • Work collaboratively with various sectors and systems including; legal, justice, law enforcement and various local, provincial, federal agencies.
  • Be able to incorporate culturally component methods in all practices
  • Be open to work with program participants across the GTA and Province of Ontario through virtual services
  • When required work in other programs/departments, such as rapid access virtual counselling for community members aged 0-30, and/or groups for men who have perpetrated abuse in their families.
Qualifications
  • Minimum MACP or MSW
  • Registered to a professional college i.e. CRPO/Registered Social Worker
  • Minimum of 3-5 years of experience working with youth, young adults, adults involved with the criminal justice system
  • Have a strong understanding of the social and mental health risk factors that lead youth young adults, and adults toward violent ideologies
  • A good understanding of geopolitics and the impact on current trends that influence people toward groups that promote acts of violence
  • A strong understanding of trauma, PTSD, and the impacts of prolonged violence exposure on people
  • A strong understanding of how racism, systems of oppression, social exclusion can lead to social isolation that may contribute to violent behavior
  • Have a good understanding of the link between hate, polarization and how it can be a predictor to violent acts
  • Have a strong operational knowledge and experience with the excision of Trauma Focused CBT, Motivational Interviewing, Narrative Therapy, SFBT, Family Systems Therapy and be able to integrate models to meet the needs of program participants as needed
  • Have an above average knowledge of forensic social work
  • Be able to assess threat to the community, self and others
  • Have demonstrated the ability to work in an inter-disciplinary team in the past
  • Has a proven ability to engage with hard to reach people and communities and is able to sustain and be persistent through creative engagement strategies
  • Presentation skills are above average
  • Enthusiasm and skill working to support change with a multi-challenged, diverse client population
  • Excellent oral and communication skills
  • Superb interpersonal and organizational skills
  • Be able to set boundaries with workload and have a strong self-care network.
  • Comfortable attending court and other legal proceedings
  • Has demonstrated the ability to manage sensitive information and develop communication strategies to message according to alignment with risk level or opportunity
  • Is confident communicating with the Director of Service/Executive Director and law enforcement agencies regarding potential issues of risk.
  • Has demonstrated the ability to have transparent communication with colleagues
  • Ability to work flexible hours including evenings (possibly two evenings per week) and weekends are essential
  • Speaking a language beyond English is an asset, to be considered; Standard Arabic, Swahili, Pashto, or Dari.
  • Has completed Violence Threat Risk Assessment (VTRA) Level one
  • .Vulnerable Sector Screening is required.
  • Access to a reliable vehicle is a definite asset as travel across the GTA will be required
