Transitional Aged Youth / Host Family Home Coordinator - Sunbeam Community & Developmental Services

Sunbeam Developmental Resource Centre – Waterloo Region (SDRC) [formally known as Developmental Services Resource Services (DSRC)] is the single point of access to developmental services for children in the Region of Waterloo.  SDRC provides service coordination and clinical services to children, youth and adults with Intellectual Disabilities and/or an Autism Spectrum Disorder (ASD), living in Waterloo Region. We are presently inviting applications for the full-time permanent position of Transitional Aged Youth Service Coordinator / Host Family Home Coordinator. The primary hours of work are 8:30 a.m. – 4:30 p.m. Monday – Friday but the successful candidate will be required to work some evenings and weekends.  Extensive travel is required throughout Central West Region and beyond.

 

The successful candidate will have experience:

  • Working with individuals with an intellectual disability
  • Working with youth 16-25 years of age
  • Coordinating services for Transitional Aged Youth
  • A sound working knowledge of the Child Welfare System
  • Familiar with working with individuals involved in the Justice System
  • A comprehensive understanding of Developmental Services Ontario
  • Working within the Transfer Payment Sector of MCCSS
  • Budget management and oversight
  • Working knowledge of Quality Assurance Measures required in group homes and Host Family Homes
  • Writing Individual Support Plans and assisting individuals to set goals who are living in Host Family Homes   

 

Qualifications:

  • Degree in Social Services or a related field
  • Familiar with the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act/Child and Family Services Act/Personal Health Information Act/and various other federal and provincial legislations, directives and guidelines, corporation bylaws, policies and procedures
  • Familiarity with the special challenges presented by a Dual Diagnosis
  • Experience working with agencies and families
  • Excellent consulting and oral/written communication skills
  • Two years of experience using a consultative model in a community setting
  • Experience in direct case management  
  • Emergency First Aid and CPR required (or must be prepared to obtain)
  • Valid driver’s license, clean driving record and reliable transportation
