We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and east downtown area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, housing assistance, harm reduction and homeless programs, Pathways to Education for high school students, infant and child development programs, parenting education and family advocacy.  We currently have the following opening on our Clinical team.

                                               Registered Nurse – Intake & Screening

                                                     Full time 35 hours per week

                                                        Including some evening hours

                        Salary Range: $33 to $40 per hour plus benefits including HOOPP pension

Position Summary

The Intake Nurse (RN) assists in the provision of direct patient care and community services in order to improve the health of the Regent Park community. As a member of an interdisciplinary clinical team reporting to the Manager, Primary Care, the Intake Nurse facilitates access to primary care services for the communities served by the Regent Park CHC. Using a client-centred and trauma-informed approach, the Intake Nurse will work with new clients through the intake and orientation process, and triage and support existing clients with urgent clinical needs to access care as appropriate. Particular attention will be paid to clients from marginalized communities who face barriers to health care access, such as people who are non-insured, immigrants or refugees; Indigenous peoples; LGBTIQ2S community members; people who experience homelessness; people who use substances and/or live with addictions and mental health challenges.

Responsibilities and Tasks Include:

Intake and Orientation

  • Screens prospective clients for eligibility for primary care services and assists clients in registering for primary care services
  • Ensures all necessary documentation is completed and stored securely
  • Orients new clients to primary care services, and connects them with other RPCHC services and programs as appropriate
  • Assigns health care teams based on their clinical and social complexity, urgency of need for service, and provider availability and area of expertise
  • Issues and tracks release of information requests
  • Conducts initial clinical assessments, and documents client history in electronic medical record
  • Acts as main intake liaison for other referring RPCHC programs and partner agencies
  • Provides information about and/or refers individuals not eligible for RPCHC services to other primary care services
  • Manages primary care waitlist and produces monthly access to care reports
  • Works with the primary care team and other internal stakeholders to streamline intake processes to facilitate greatest access to primary care and ease of experience for clients

Screening

  • Screens and triages clients who walk in or call for urgent clinical support
  • Point of contact for other RPCHC program staff working with primary care clients who require urgent clinical support 
  • Screens and assesses clients with minor episodic illnesses or chronic diseases according to established protocols
  • Provides supportive and therapeutic counselling and crisis intervention to clients as needed on such issues as sexuality, family planning, pregnancy choices, family violence, lifestyle
  • Assists with referring clients to specialist, hospital or community agencies
  • Advocates on an individual basis for and with clients to other agencies
  • Performs basic nursing skills (e.g. dressings injections, suture/clip removal and assisting with other procedures as required)
  • Performs added nursing skills (according to College of Nurses) as required such as venepuncture, pulmonary function testing, ECGs, maintaining the confidentiality, accuracy, and quality of patient information and accurate electronic health records

Required Skills

  • Current certification as a Registered Nurse in the Province of Ontario required.
  • Minimum of 2 years’ nursing experience is required, in a community setting or combination of community and hospital or public health settings.
  • Experience in primary care is an asset
  • Experience working with, or a strong desire to build a practice in, working with people who use substances;
  • Experience working with marginalized communities experiencing barriers to accessing health care is an asset.
  • Demonstrated understanding of pharmacology, clinical theories and practices
  • Extensive knowledge and understanding of harm reduction principles and practices, substance use and overdose response
  • Experience using electronic medical records (Nightingale or PS Suites an asset)
  • Strong communication skills (verbal, listening and written).
  • Organizational and time management skills
  • Demonstrated abilities in conflict management, problem solving and interpersonal relations.
  • Understanding of anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
  • Ability to work collaboratively within and across teams
  • Knowledge of the Regent Park area and issues affecting the community
