35 hrs/wk, Weekdays, and occasional evening and week-ends

ACSA is a non-profit, multi-service agency addressing needs and empowering children, youth, newcomers, homeless, and otherwise underserved communities. Our core programs include housing and outreach programs, food security programs, newcomer services, youth outreach and therapy programs, child and family programs, early years’ programs, computer access programs, and community engagement.
The Home for Good Case Manager will provide intensive case management support to housed adults and families. Activities including housing stabilization, eviction prevention services and re-housing when required. The Case Manager will work within a Housing First, client-centred, anti-oppression and harm reduction approach to support clients with complex needs to maintain housing and to be linked with other support services, as determined by each individual client.
The Moderate Support Case Manager will maintain a ratio of 1:20 clients who will receive intensive supports with regular contacts.
- Provide case management supports to clients using a strengths-based framework with clients to develop service planning and linkages, goal setting, and supports to stabilize and sustain tenancies.
- Receive and follow up on referrals according to City of Toronto Coordinated Access processes; follow all Home for Good requirements.
- Maintain a caseload to provide individualized and flexible case management services, through a housing-first approach.
- Assist clients to identify and achieve goals in relation to housing, income supports, substance use, physical and mental health, food security and other service needs as defined by each client.
- Work with clients to access and manage social housing applications.
- Assist clients towards integrating into the community, and empowering their use of resources and services to maximize their independence and potential.
- Provide informed, managed referrals to other services including Rent Bank and utility programs, legal services, Toronto Employment and Social Services offices, and other agencies.
- Facilitate follow-up meetings in community to implement action plan, assess progress of goals and explore new challenges.
- Provide harm reduction counselling to identify strategies that reduce negative impacts of substance use on tenancies.
- Accompany clients to appointments, including Landlord and Tenant Board hearings as required.
- Work with clients to complete community mapping, crisis plans, discharge planning.
- Facilitate access to services within 4155 Sheppard Avenue East and Dorset Park Hub locations, through meetings and collaboration with staff and agency partners
- Complete service linkages and referrals to ACSA’s income tax clinic, medical clinics, Drop-In, food bank, along with other service providers within case management team, ensuring easier access points to services.
- Build and maintain responsive relationships with housing providers to troubleshoot and support the resolution of tenancy issues, mediating when required.
- Maintain accurate case management records and complete statistical and qualitative reporting as required by funder and supervisor.
- A college diploma or a degree in human services or equivalent.
- Two years’ experience providing intensive case management services to clients with complex needs that are homeless or at risk of homelessness.
- Demonstrated understanding of housing, health, trauma, substance use, financial, legal, and other issues that impact clients’ lives.
- Strong understanding of market rent and subsidized housing systems and policies, keeping up-to-date on emerging trends and changes; knowledge of the RTA.
- Demonstrable understanding and commitment to harm reduction framework and housing-first approach.
- Ability to work independently and as a member of a team, and to manage time well.
- Ability to manage issues tactfully, and relationships with professionalism, and resolve conflicts.
- Experience with maintaining statistics, writing program reports and developing and implementing evaluation tools.
- Strong written and oral communication skills.
- Crisis Intervention, First Aid and CPR certification.
- Competency in Microsoft Word, Outlook and Excel, as well as data reporting programs.
- Knowledge of community resources for homelessness, health, mental health and substance abuse.
- Experience working with diverse communities.
- Strong organizational and time management skills.
- Strong crisis management skills.
- Experience working with partners.
- Strong writing and communication skills.
- Proficiency in Microsoft Word, Excel, and Power Point.
- Valid driver’s licence, clear driving abstract and a vehicle for daily use
- Valid Police Records Check.
The Home for Good Case Managers work in the community, meeting with clients in coffee shops, public places and in the client’s home. The Case Manager also works from an office at 4155 Sheppard Avenue East and attends meetings and appointments anywhere in the City of Toronto as required. (Covid 19 adjustments have been made to Working Conditions).
