We are hiring Case Managers - Housing and Homelessness!
As we continue to grow our team, we are looking for three Case Managers to join our passionate, dedicated, and “BRAVE” team! - TWO (2) Permanent: Full-time, 35 Hours per Week;
- TWO (2) Contract: Full-time (6 months ending March 31, 2021), 35 Hours per Week;
- ONE (1) Permanent: Part-time, 17.5 Hours per Week

Location: In accordance with current public health directives, training will be available both remotely (i.e., via teleconference and videoconferencing technologies) as well as onsite while respecting physical distancing. During the current COVID-19 outbreak, work can be completed primarily from home with equipment provided by CMHA. Once public health directives are lifted, the Case Managers will be working primarily in our Aurora Office (15150 Yonge Street, Aurora, ON L4G 1M2) and will be required to travel within York Region & South Simcoe.
The Case Manager works within a Housing First model and collaborates with clients to provide individualized and flexible support to help them find housing, engage and integrate them into the community, and helps to facilitate their access to resources and supports to maximize their independence and potential, and promote their recovery. The work with clients is achieved with a focus on Housing stabilization.
Individuals are assisted to identify strengths and needs and develop long term goals, short term objectives to meet these goals, and concrete action steps to achieve the objectives. In addition, the Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability. The Case Manager may be assigned other duties as required.
In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.
The Case Manager may be assigned to different programs as required based on need (i.e.: ACTT, Case Management, etc…)
• A degree or diploma in the health or social services.
• Current registration with the OCSWSSW or another relevant regulatory body under the Regulated Health Professions Act; cannot be in the qualifying stages.
• Degree plus four years or more experience in a community-based organization working with adults with serious mental illness in the community.
• Diploma plus six years or more experience in a community-based organization working with adults with serious mental illness in the community.
• Experience working with people experiencing homelessness is preferred
• Experience working with individuals experiencing serious mental illness, concurrent disorders and dual diagnosis (developmental disability and/or traumatic brain injury) is preferred
• Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
• Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with clients
• A second language (French, Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred.
• A valid Canadian Full G driver’s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
• A satisfactory Vulnerable Sector Screening (Police Check)
