Location: Aurora, York Region
The Case Manager collaborates with clients to provide individualized and flexible support to help them find housing, engage and integrate them into the community, and helps to facilitate their access to resources and supports to maximize their independence and potential, and promote their recovery.
The Case Manager works in partnership with clients to provide individualized and flexible services to engage, support, and integrate the clients into the community, and facilitate their use of resources and supports to maximize their independence and potential. Individuals are assisted to identify strengths and needs and develop long term goals, short term objectives to meet these goals, and concrete action steps to achieve the objectives. In addition, the Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability. The Case Manager may be assigned other duties as required.
In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.
This position reports to and is supervised by the Manager that oversees the Supports within Housing Program.
The Case Manager may be assigned to different programs as required based on need (i.e.: ACTT, Case Management, etc.)
- A degree or diploma in health or social services
- Current Registration with the OCSWSSW or a regulatory body under the Regulated Health Professionals Act
- Degree plus four years or more experience in a community-based organization working with adults with serious mental illness in the community
- Diploma plus six years or more experience in a community-based organization working with adults with serious mental illness in the community
- Experience working with individuals experiencing significant mental health and addictions challenges and people experiencing homelessness. Familiarity with the “Housing First Model” would be an asset
- Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
- Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with clients
- A second language (French, Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred
- A valid Canadian Full G driver’s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
- A satisfactory Vulnerable Sector Screening (Police Check)
Office work, community settings, meetings in clients’ home and travel with clients.
